Transplant Coordinator

The Transplant Coordinator is responsible for managing and coordinating all aspects of the organ transplantation process, ensuring continuity of care for both donors and recipients. This includes pre-transplant evaluation, coordination of the transplant procedure, and post-transplant follow-up, working closely with patients, families, and multidisciplinary medical teams

Role & responsibilities

• Conduct pre-transplant interviews and assessments of patients and families.
• Interpret eligibility criteria and educate patients and families about the transplant process, risks, and post-operative care
• Collaborate with surgeons, physicians, nurses, and external agencies to facilitate the transplantation process from donor identification to organ allocation and surgery.
• Coordinate pre-transplant preparations, including scheduling diagnostic tests and gathering medical/social histories.
• Ensure all legal and documentation requirements are met, including obtaining informed consent
• Provide support and follow-up care to recipients and their families after transplantation, including monitoring for complications and arranging follow-up appointments
• Educate patients on medication management and lifestyle modifications post-transplant
• Serve as the primary point of contact for patients, families, and the healthcare team throughout the transplant process
• Maintain accurate records and prepare written summaries for patient files and regulatory compliance

Preferred candidate profile

• Bachelors degree in nursing, life sciences, or related field.
• Minimum 1 year of experience as a transplant coordinator
• Strong organizational and time-management skills; ability to manage multiple cases at different stages.
• Excellent communication, counseling, and interpersonal skills.
• Knowledge of relevant healthcare regulations and documentation practices.
• Ability to work collaboratively in a multidisciplinary team environment.
• Compassion, empathy, and the ability to support patients and families through complex and emotional situations
