Rehabilitation Support Worker – Community Hospital Discharge Team - Health & Community Care

The shift pattern for this post is:

Monday - Friday, 08:30 - 16:30We are seeking a dedicated Rehab Support Worker to join our hospital discharge team. This role is integral to supporting individuals returning home after a hospital stay, and supporting them with their rehab goals and working closely with our Occupational Therapists and Physiotherapists within the Integrated Team.
