Job description

• Identify and diagnose psychological, emotional, or behavioral issues;
• Develop and implement treatment plans and therapeutic processes;
• Assesses patients and make recommendations on admission and discharge and on therapeutic activities;
• Maintain an open line of communication with care providers, patients and their family members in order to efficiently address emergencies;
• Assessing clients' risk, considering their mental health and social needs when performing interventions;
• Monitor client progress through regular meetings and checkups;
• Collaborating with other healthcare providers to evaluate the medical and physical condition of clients.
• Help clients define goals and plan action to achieve personal, social, educational, and vocational development and adjustment;
• Ensuring the maintenance and safe storage of accurate program and client records, and all other pertinent documents.
