Night Supervisor

This initiative aims to:

• Improve response time,
• Minimize clinical risk, and
• Strengthen remote supervision during night hours.

Expectations from On-ground Nursing Teams:

• Record and enter

  vital signs (BP, HR, RR, Temperature, SpO)

  and any other required clinical data every

  2 hours

  in the shared Google Sheet.
• Ensure

  accuracy and timeliness

  of entries.
• Respond promptly to the Night Supervisor's calls or messages in case of

  delays, alerts, or escalations

  .

Responsibilities of the Night Supervisor:

• Review data in

  real-time

  .
• Follow up immediately on any

  missing, delayed, or abnormal vitals

  .
• Escalate any unresolved issues to the

  assigned Medical Officer or Unit Nursing Officer

  .
• Address

  equipment-related issues

  by guiding the nurse through a

  video call

  and attempting resolution.
• Share a

  daily shift report

  with the

  Corporate Clinical Manager (CCM)

  .
• Ensure

  zero lapses

  in vital sign documentation for all assigned patients.

Scope of service:

• Night supervision is applicable for all critical and tracheostomy patients and for any patients who had specific requirements as ordered by the unit physician / nursing officer basis the criticality of the patient.
• Monitoring non- critical patients can come at a certain cost to the patient.
• This service can be given to the patients who specially opt for this service.
• Maintain a shift-wise log of:
• Non-compliance cases
• Clinical escalations
• Interventions provided

Operational Metrics(Performance & Compliance Monitoring)

• Definition

  : Percentage of patients whose vitals are updated every 2 hours.

• Target

  : 95% per shift.

• Measured by

  : Timestamp audit from the Google Sheet.

1. Response Time to Missing/Delayed Entries

• Definition

  : Time taken by the supervisor to alert the nurse after identifying a missed entry.

• Target

  : 20 minutes.

• Measured by

  : Alert logs and time stamps.

2. Follow-Up Closure Rate

• Definition

  : Percentage of alerts resolved within the same shift.

• Target

  : 90%.

• Measured by

  : Supervisor follow-up log.
