Job Description

We are looking for a highly skilled and compassionate Member Care Assistant to join our team at All Peoples Church World Outreach. The ideal candidate will have excellent communication skills and the ability to provide top-notch care to our members.
Roles and Responsibility
  • Provide individualized support to members, including daily check-ins and regular follow-ups.
  • Assist with daily tasks such as grocery shopping, meal preparation, and medication management.
  • Develop and implement personalized care plans tailored to each member's unique needs.
  • Collaborate with other departments to ensure seamless coordination of care services.
  • Maintain accurate records of member interactions and progress notes.
  • Participate in ongoing training and professional development to stay current on best practices in member care.
Job Requirements
  • Minimum 2 years of experience in a related field, preferably in healthcare or social work.
  • Strong knowledge of patient-centered care principles and practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize tasks.
  • Strong problem-solving and critical thinking skills.
  • Proficiency in Microsoft Office and other software applications.
