Medical Transcriptionist

• Transcribe medical dictations from doctors and healthcare professionals into accurate written reports.
• Prepare patient records including history, discharge summaries, operative notes, consultation notes, and diagnostic reports.
• Review and edit transcribed documents for accuracy, grammar, and medical terminology.
• Ensure correct use of medical abbreviations and terminology.
• Maintain confidentiality of patient information as per hospital policies and HIPAA guidelines.
• Proofread documents to ensure clarity, consistency, and completeness.
• Update patient records in Hospital Information System (HIS) or Electronic Medical Records (EMR).
• Identify and correct inconsistencies, missing information, or unclear dictations by coordinating with doctors.
• Follow turnaround time (TAT) standards for report submission.
• Maintain organized digital and physical medical records.
• Comply with hospital policies, quality standards, and legal requirements.
• Assist in maintaining accuracy for billing, coding, and insurance documentation.
• Stay updated with new medical terminology, procedures, and transcription guidelines.
• Report errors or discrepancies in medical documentation to supervisors.
• 02-03 years experience for store in hospital.
