Job Description

Assess the psychosocial needs of patients and their families by conducting interviews, evaluations, and case histories to identify emotional, social, financial, and environmental factors affecting treatment and recovery.
Provide counseling, guidance, and emotional support to patients and caregivers, helping them cope with illness, hospitalization, treatment plans, and lifestyle adjustments while ensuring confidentiality and ethical practice.
Coordinate with doctors, nurses, therapists, and external agencies to develop and implement comprehensive care plans, facilitate referrals, arrange community resources, and support continuity of care post-discharge.
Maintain detailed case documentation, prepare reports, participate in multidisciplinary meetings, ensure compliance with healthcare policies, and educate patients and families about healthcare rights, benefits, and available support services.
