The Medical Social Worker will be responsible for supporting patients and their families by providing social services, coordinating discharge planning, and performing case management tasks. They will work collaboratively with healthcare professionals to assess patient needs, develop care plans, and connect patients to community resources to ensure holistic care and improved outcomes. Qualifications Experience in Medical Social Work and Social Work to support patients and families effectively Proficiency in Discharge Planning and Case Management for seamless patient transitions Understanding of Social Services and the ability to connect individuals to community support programs Strong interpersonal and communication skills to collaborate with patients, families, and healthcare teams Knowledge of healthcare policies, ethics, and patient rights, with a focus on compassionate care Master's degree in Social Work or a related field is preferred Experience working in a healthcare setting or with diverse populations is an advantage
