Job Description

The Medical Coder is responsible for accurately reviewing clinical documentation and assigning appropriate medical codes in compliance with industry standards, client-specific guidelines, and organizational policies. The role plays a critical part in ensuring high-quality coding output that meets productivity, accuracy, and turnaround time requirements.
The Coder reviews medical records, including physician notes, operative reports, discharge summaries, and diagnostic results, to extract relevant clinical information. Based on the documentation, the Coder assigns accurate ICD-10-CM, CPT, HCPCS, and/or DRG codes as applicable to the specialty and client process. Strict adherence to official coding guidelines, payer policies, and client-specific instructions is mandatory.
The role requires maintaining defined accuracy and productivity metrics while meeting internal and external SLAs. Coders are expected to self-audit their work, identify potential errors, and implement corrective actions to minimize rework and quality deviations. Active participation in quality feedback sessions, calibration meetings, and refresher trainings is required to continuously enhance coding accuracy.
The Medical Coder responds to QC feedback in a timely manner, ensures error correction as instructed, and incorporates learning to prevent recurrence of similar errors. Coders are also expected to raise documentation gaps or coding-related queries through established query processes to ensure compliance and clarity.
Strict compliance with organizational policies, data security standards (HIPAA), and process guidelines is essential. The role requires effective time management, attention to detail, and the ability to work independently while contributing to team goals.
