Health Information Management (HIM)

Roles and Responsibilities:

• Review and verify patient health records for accuracy, completeness, and legibility.
• Abstract relevant medical information from patient records for coding and data entry.
• Assign appropriate diagnosis and procedure codes (ICD/CPT) as per training and organizational guidelines.
• Maintain confidentiality and ensure compliance with HIPAA and other data protection standards.
• Assist in organizing, digitizing, and maintaining electronic medical records (EMR).
• Generate and update reports related to patient data and documentation.
• Coordinate with clinical, billing, and administrative teams to resolve discrepancies in documentation.
• Support internal audits and quality checks for data accuracy and completeness.
• Participate in regular training sessions to stay updated with coding and healthcare data standards.
• Ensure timely completion of assigned tasks within defined quality parameters.
• Assist in data validation and ensure accurate entry of patient demographic and clinical details.
• Support process improvement initiatives within the HIM department.
• Ensure adherence to organizational policies, documentation standards, and data security protocols.
• Collaborate with team members to achieve daily productivity and quality targets.
• Maintain proper filing and retrieval systems for physical and electronic medical records

Preferred Candidate Profile:

• Fresh graduates in Life Sciences (B.Pharmacy, M.Pharmacy, B.Sc/M.Sc in Biotechnology, Microbiology, Biochemistry, etc.).
• Good communication and analytical skills.
• Basic understanding of medical terminology and healthcare documentation (training will be provided).
• Attention to detail and ability to maintain accuracy in data handling.
• Proficiency in MS Office tools.
