Role Overview

We are seeking Part-Time Online Diabetes Educators to provide education and counseling to individuals living with or at risk of diabetes. You will conduct online sessions focused on self-management, lifestyle modification, and adherence support, helping patients improve their health outcomes. The role also involves developing personalized care plans, monitoring progress, and collaborating with healthcare professionals to ensure comprehensive care.

Key Responsibilities

• Conduct virtual one-on-one consultations for diabetes management.
• Provide guidance on healthy eating, physical activity, medication adherence, and glucose monitoring.
• Support patients and families in managing day-to-day challenges of diabetes.
• Create personalized, practical care plans tailored to patient needs.
• Offer ongoing motivation and follow-up for sustainable lifestyle changes.
• Maintain accurate records of sessions and patient progress in HopeQure's system.
• Participate in online awareness programs, workshops, or group sessions.

Qualifications & Skills

• Bachelor's/Diploma/Certificate in Diabetes Education is mandatory
• 5+ years of experience in diabetes education, patient counseling, or clinical care.
• Knowledge of dietetics, diabetes care, and lifestyle management.
• Candidates living in Noida will be considered for full time role.
• Candidates with prior experience of corporate setup and having worked with copmanies where they coordinated Diabates program will be preferred.
