As a healthcare professional in this role, your primary responsibilities will include studying patients" medical conditions and designing personalized care plans. You will be tasked with conducting care team meetings to ensure effective coordination and communication among team members. Internal monitoring of data will be crucial, and you will need to suggest tactics to enhance care quality and achieve practice goals. Analyzing patient data recorded in software will be a key aspect of your role, particularly in defining care plans for high-risk chronic disease patients. Evaluating the risk level of the patient population based on their medical conditions will also fall within your purview. Effective communication skills will be essential as you liaise with doctors" offices via phone calls or email to coordinate patient care seamlessly. Attendance at virtual learning sessions will be mandatory to enhance your project knowledge and disseminate relevant information within the team. Your educational background should ideally include being a graduate nurse, medical officer, or having another medical-related background. Proficiency in using computer and software tools is a must, and while it's preferable for candidates to be students preparing for the USMLE, it is not mandatory. Strong verbal and written communication skills, coupled with a quick learning aptitude, will be highly valued in this role.
