Job description

Key Responsibilities:

• Monitor patients remotely through the use of digital tools and technologies, ensuring they are

following their care plans effectively.

• Provide patient education and support regarding their chronic conditions, treatment regimens,

and lifestyle changes.

• Coordinate with healthcare providers, including doctors, nurses, and other specialists, to ensure

comprehensive care delivery.

• Assist in assessing patient progress, identify potential issues, and recommend appropriate

interventions.

• Review and analyze patient data (e.g., vitals, symptoms) to identify trends and changes in

condition.

• Ensure timely follow-ups with patients, ensuring adherence to treatment plans and addressing

concerns.

• Maintain accurate patient records and documentation for continuous care and reporting.
• Communicate effectively with patients and caregivers to provide emotional support and health

advice.

• Stay up-to-date with advancements in healthcare technology and chronic care management

practices.

Eligibility Requirements:

• Education:
• BSc Nursing, Pharmacy, Pharm D or Hospital Management degree required.
• Skills:
• Excellent communication and interpersonal skills, both verbal and written.
• Ability to use digital health tools and remote monitoring technologies.
• Ability to work independently and collaboratively within a remote team.
• Empathetic, compassionate, and patient-centered approach.

Preferred Qualifications:

• Additional certifications in chronic care management, patient coordination, or telemedicine are a

plus.

• Experience with electronic health records (EHR) systems or telemedicine platforms.
