Unit Specific Protocols Meet the patient and family on arrival to the unit. Send appropriate referrals and communicate with consultants. Ensure completion of patient records at the time of discharge. Respond promptly to any nurses urgent request for help in caring for any patient. Respond promptly to “Code Blue” announcements. Resuscitation through Risk Stratification for all ages (Basic Life Support/Paediatrics Advance Life Support /Neonatal Advance Life Support). Comply with all Hospital Policies and procedures. Display professional behavior with patients, colleagues and other Hospital staff. Take history and perform examination of the patient. Male doctors shall ensure presence of a nurse or female colleague if examining a female patient. To monitor the patients on invasive ventilation. To take charge of the patients from the resident on previous duty. Inform the patient’s condition to consultant SOS. To assist in OPD. Ensure the plan of care for the patients under their care. To send investigations as when as required. To discuss the condition with the treating doctor as when as required. Assist in Specialized procedures Insert peripheral intravenous catheters when required. NG tube insertion/U cath Insertion. Insert invasive vascular access lines only under supervision. Carry out invasive diagnostic procedures such as paracentesis. Take appropriate blood samples in case of difficulty by phlebotomist or nurses Patient and family education To maintain the confidentiality of patient. To provide patient & family education. To provide diagnosis specific patient and family education. Documentation Document history & physical examination within 24 hours of admission Write daily progress notes at least once in 24 hours and more frequently if patient condition requires more frequent observations. Shifting and receiving notes. To prepare discharge summaries for the patients under their care. Write all orders in legible writing in the “Physician Order Sheet”, after confirmation with a senior colleague or Consultant. Follow up on investigations sent and communicate results to the consultant. Document such communications and the resultant necessary orders. Participation in Continuous Quality Improvement. To formulate the Process Improvement programs and carry on Quality Initiatives in the department. To perform all other jobs as may be assigned due to exigencies of work.

To ensure that the IP rounds are duly done and the patients are attended to empathetically. To comply with patient protocols (OP-IP/OT/ Diagnostics/ ICU/ Triage) & document patient File/ Prescriptions/requisitions (legible/ complete). To ensure MLC notification / documentation and statutory compliance. To ensure Documentation at the time of Discharge. To complete all OP & IP records on time with accuracy. To ensure documented handover at the end of shifting/ Transfer of patients to another unit. To ensure proper Bio Medical Waste Management & follow Infection Control protocols. To follow ethical Medical practice and to ensure clear and prompt communication with the Admitting as well as Referral Consultants. To be well groomed, punctual & adhere to company policies and practices. To have complete orientation of IMS and NABH system. To have complete Awareness about Disaster Management Programme (DMP) of the hospital. To Comply with patient safety policy.

To ensure proper attention to the patient & their attendants. To proactive in terms of patient care (Planning and anticipation) To provide expert consultation and manage all patients related to obs & gynae . To keep abreast with latest research, development & techniques in the field of obs & gynae. To conduct and supervise all tests / procedures related to obs & gynae. To generate reports on test / procedures performed (Operation notes and discharge summary). To ensure that the wards / O.T. rooms have equipment / facilities necessary for rendering patient care. To supervise all nursing / para medical and non medical staff of the department to maintain discipline and high standard of medical care at the departments. To select type and nature of medical consumables and equipment to be procured. To co-ordinate and contribute with the team for development of newer facilities To co-ordinate with Head Of Department on all matters relating to:- provision infrastructure and maintenance support-complaint redressal of patients. To ensure optimum utilisation of resources by the him /her and subordinates. To be well groomed, punctual & adhere to company policies and practices. To Comply with the service quality Process, environmental & occupational issues & policies of the respective area. To Comply with patient safety policy. To ensure safe utilization of equipment and proper waste disposal system.
