Job Description

As a Care Coordinator in the Healthcare Domain, your role involves assisting patients in managing their overall health through daily monitoring and calls. You will be responsible for an assigned group of patients, following up with them monthly via phone. Your key responsibilities will include: - Reviewing patient recorded daily vital readings - Educating patients on improving diagnosed diseases - Assisting patients in supporting the plan of care established by their provider - Verifying and recording patients" demographics, medical issues, medications, and risk management factors to create a clinical summary using specialized medical software - Handling potentially difficult conversations calmly and professionally - Participating in individual & group projects as directed by management - Demonstrating strong communication skills to engage patients in conversation and motivate them to follow the care plan Immediate joiners are preferred for this full-time WFH position in a US Shift (06:30 PM or 04:00 AM IST). The company is located in Surat, Gujarat, and the hiring office is based there.
