Verify and record the patient’s demographics, medical issues, medications, and risk management factors to create a clinical summary using specialized medical software. Handle potentially difficult conversations calmly and professionally. Participate in individual & Group projects as directed by management. Strong communication skills – ability to engage patient in conversation, motivate patient to follow the care plan.
