Team Lead - Hospital at Home

As a Team Lead for the NHS Borders Hospital at Home team, you will have the opportunity to lead and manage a team of healthcare professionals to deliver skilled care to patients in the community setting. Your role will involve operational management and professional leadership within the Hospital at Home team, ensuring the delivery of advanced clinical care to patients referred from Acute Services. You will be responsible for ensuring that patients can be safely managed at home or preventing hospital admission for patients in the Community.
