Review and interpret operative and clinical documentation to determine appropriate CPT, ICD-10-CM, and HCPCS codes for all surgical procedures.
Assign correct modifiers and ensure codes are compliant with payer-specific and NCCI (National Correct Coding Initiative) guidelines.
Validate that documentation supports all coded diagnoses and procedures.
Query physicians or surgeons for clarification when documentation is incomplete or unclear.
Work collaboratively with the billing, compliance, and revenue cycle teams to resolve coding-related claim denials.
Stay current with coding regulations, payer guidelines, and official coding updates (CPT Assistant, ICD-10-CM, CMS, etc.).
Meet productivity and accuracy benchmarks as set by the organization.
Participate in regular coding audits and implement feedback to maintain high-quality standards.
Maintain confidentiality and adhere to HIPAA regulations at all times.
