Staff Nurse

To deliver compassionate, patient-centered nursing care to individuals with life-limiting illnesses in their homes, focusing on symptom relief, emotional support, education of caregivers, and ensuring dignity and comfort through end-of-life care.

1. Clinical Home Care: • Provide nursing assessments and implement individualized care plans for patients at home. • Administer Prescribed medications (oral, injectable, IV, SC) and symptom-relief drugs. • Manage medical devices such as feeding tubes, catheters, IV lines, and oxygen therapy.

2. Symptom and Pain Management: • Monitor and manage distressing symptoms such as pain, breathlessness, nausea, and fatigue. • Assess symptoms using validated tools and escalate issues to physicians as needed.

3. Emotional and Psychosocial Support: • Offer empathetic support to patients and family caregivers, particularly during the end-of-life phase. • Help families cope with anxiety, uncertainty, and bereavement preparation.

4. Caregiver Education and Training: • Teach families safe caregiving practices such as medication administration, turning patients, hygiene, oral care, eye care and feeding. • Provide clear instructions and demonstrations during visits.

5. Coordination and Communication: • Coordinate with Doctor & project coordinator for holistic patient care. • Communicate clearly with families regarding patient condition, care plans, and follow-up needs.

6. Documentation and Record Keeping: • Maintain accurate and timely patient care records after each home visit. • Document vitals, symptom progression, nursing interventions, and care updates. • Flag when the medicine stock is going down and keep the medicine stock record up to date • Maintain Vehicle Log book.

7. Emergency and End-of-Life Support: • Respond to emergency care needs or sudden symptom deterioration at home. • Provide end-of-life nursing support, ensuring comfort and respect during the dying process. • Call 108 for timely referral of the patient to the treating hospital or SSGH/ Govt Hospital 8. Infection Control and Safety: • Follow standard precautions and ensure safe disposal of medical waste in home settings.
