Job Description

Daily rounds of assigned areas
Overall coordination and supervision of medical care in assigned areas
To capture and escalate any medical related concerns of the patients/ Clinician
RCA of all medical complaints along with suggested CAPA
To escalate any Clinician related concerns
Monitoring discharge planning by Clinicians and analysis of gaps. Gap escalation to MS.
Getting MDTs conducted for glass list patients (identified as per category)
Clinical update of glass list and HWC patients
ALOS tracking as per package and escalation of long stay cases (More than 7 days or beyond defined package days)
Ensure tracking of Clinical Indicators of assigned areas
Analysis of package consumption and monitoring and tracking of packages with high consumption
Monitoring of Initial Assessment, Re-assessments and other documentation compliance by clinical staff.
Monitoring TAT of ER Referral and escalation of concerns
Meeting all LAMA patients to identify & resolve correctable causes.
Closure and updating of meeting minutes as assigned
Ensure timely dispatch of monthly compliance/statutory data and send update to MS by 7th of every month
Any other work as assigned by MS
