Job Summary:

The MRD Technician is responsible for maintaining, organizing, and safeguarding patient medical records in compliance with hospital policies, legal requirements, and NABH standards. In a maternity hospital, this role also involves handling the registration of births and deaths, issuing birth certificates, and ensuring timely reporting to local authorities.

Key Responsibilities:

• Maintain and update patient medical records (inpatient, outpatient, maternity, and newborn records) systematically.
  
• Register all births, stillbirths, and deaths as per statutory requirements and hospital policy.
  
• Coordinate with local municipal / statutory authorities for timely submission of birth and death reports.
  
• Issue birth certificates to parents and maintain proper records for the same.
  
• Ensure completeness, accuracy, and proper filing of all medical documentation, including delivery records, consent forms, and discharge summaries.
  
• Assign and manage medical record numbers (MRN) for all patients and newborns.
  
• Maintain confidentiality and security of patient data at all times.
  
• Support coding and indexing of diseases/procedures as per ICD standards.
  
• Assist in medico-legal record documentation and preservation.
  
• Archive old medical records as per retention policy.
  
• Prepare and submit MRD-related statistics and reports to management.
  
• Provide support during hospital audits, NABH inspections, and medico-legal cases.
  

Key Skills & Competencies:

• Knowledge of medical terminology, especially in obstetrics, gynecology, neonatology, and medico-legal documentation.
  
• Familiarity with ICD coding, hospital record management systems, and statutory requirements for birth/death registration.
  
• Strong organizational and record-keeping skills.
  
• Attention to detail and accuracy in documentation.
  
• Good communication and coordination skills with doctors, nurses, parents, and government authorities.
  
• Computer proficiency (Hospital Information System / EMR software).
  

Qualification & Experience:

• Diploma / Certification in Medical Record Technology (MRT) or Health Information Management.
  
• 13 years of experience in MRD, preferably in a maternity / multi-specialty hospital.
  
• Experience in handling birth/death registrations and certificate issuance is highly desirable.
  
• Knowledge of NABH standards in medical records management is an added advantage.
