Project Brief Hand in Hand India has signed up MOU with The Life Foundation CSR Partner for improving access to NCD screening and treatment through collaboration on time bound targeted approaches. Comprehensive Primary Health Care (CPHC) bringing healthcare closer to the homes of people covering healthcare services for non-communicable diseases, including free essential drugs and diagnostic services with focused on evidence-based interventions in NCD screening treatment adherence, follow up and further higher speciality expert opinion, treatment and rehabilitation in case of advanced stage of disease as per Ayushman Bharat - Health and Wellness Centre. The aim of this project initiative is advancing and sustaining high and equitable NCD screening, coverage in vulnerable geographies, with high impact Health Promotion, Health Education activities to strengthen comprehensive primary health care services. Job Summary Nursing Assistant - will be worked at community level with in the catchment area of any PHC of selected block in the selected district - Howrah. He/ She will be immediate reporting to Staff Nurse placed by Hand in Hand India at PHC and collaborate with NCD SN -MLHP (Mid-Level Healthcare Provider) and the district NCD project coordinator of HIH. Their primary role will be to handle screening crowd management at field level, Fill the gaps and support services for PHC, mobilisation of high risk patients for screening to PHC, vital collection in the absence MTM Nurse, distribution NCD medicines review lab reports on need basis, coordinate patients with NCD staff nurse at PHC, Target would be average of 100 patients per day further data collection reporting systems to be completed daily basis. Specific responsibilities: Nursing Assistant is expected to perform the following specific duties, which include but are not limited to: Identify the people in the 18 years and above in the community and will do the NCD risk assessment Mandatorily mobilisation of patient in the age of 30 years and above and motivate them to go to nearby PHC for NCD diagnosis Perform NCD vital examination with support of MTM Team such as height, weight, BMI, RBS, BP and data entry to reporting systems and patient counselling, developing health promotion, health education activities. Visit the patient each month for ensuring compliance, checking on diet and life style modification, and measuring the blood pressure/ blood glucose at field level. Assist in Staff Nurse at PHC for screening protocol adherence and NCD Diagnosis. Referring those who are suspected of any of the NCDs to the Primary Health Centre (PHC). Follow- up of those who are diagnosed with any of these NCDs and ensuring that they adhere to the treatment plan. Identify the warning signs of complications and refer to appropriate facilities Coordinate with NCD District Coordinator for maintaining records and registers as necessary Supportive supervision – joint visit with community mobiliser/Women Health Volunteers in the community Make individual patient NCD card with unique ID at facility level Anthropometry of individuals comes with Risk assessment comes with Home visit Referral to PHC for investigation and treatment Attend NCD training and updates with District level officials Conduct mass NCD health education activities to the patients. Essential qualifications Diploma in Axillary Nursing Midwifery or Diploma in Health Assistant, Diploma in Nursing Assistant preferable.
