General Physician for night shift

As a Geriatrician in this role, your main responsibilities will include: - Providing Comprehensive Medical Care: - Diagnosing and treating common age-related conditions like cardiovascular disease, diabetes, and dementia. - Managing complex medical conditions that require multiple medications or interventions. - Offering preventative care and health promotion to help older adults maintain their health and well-being. - Conducting Geriatric Assessment: - Performing comprehensive assessments to understand an older adult's overall health, including their physical and cognitive function, social and emotional well-being, and medication list with potential drug interactions. - Prioritizing Patient-Centered Care: - Understanding the patient's goals and preferences for treatment. - Collaborating with the patient and their family to develop a personalized care plan. - Addressing not only the medical conditions but also the broader impact on the patient's life and quality of life. - Coordinating Care: - Working as part of a multidisciplinary team to coordinate care with other healthcare professionals such as primary care physicians, specialists, social workers, therapists, and nursing home staff. - Advocating and Providing Support: - Advocating for patients" rights and needs to ensure they receive the best possible care and support. - Assisting patients in navigating the healthcare system, accessing resources, and making informed decisions about their care.
