DOCTOR OF PHARMACY

JOB DESCRIPTION – Doctor of Pharmacy

Position: Clinical Pharmacist / Pharm.D
Department: Pharmacy
Location: AL AZHAR MEDICAL COLLEGE & SUPER SPECIALTY HOSPITAL THODUPUZHA

Key Responsibilities:

• Review and validate medication orders for accuracy, safety, and appropriateness.
• Provide clinical pharmacy services including medication therapy management and drug utilization review.
• Monitor patient progress, identify drug-related problems, and recommend therapy modifications.
• Participate in clinical rounds with the medical team to provide pharmacotherapy insights.
• Counsel patients on medication usage, dosage, side effects, and adherence.
• Ensure proper documentation of interventions and maintain medication records.
• Collaborate with physicians, nurses, and other healthcare staff to optimize patient outcomes.
• Oversee storage, handling, and dispensing of medications as per hospital policies and regulatory standards.
• Participate in antibiotic stewardship and pharmacovigilance programs.
• Conduct medication audits, drug information services, and assist in formulary management.
• Provide training and guidance to pharmacy interns and junior staff.

Required Qualifications & Skills:

• Pharm.D (Doctor of Pharmacy) from a recognized institution.
• Strong knowledge of pharmacotherapy and clinical pharmacy practices.
• Excellent communication and patient counseling skills.
• Ability to work effectively within a multidisciplinary healthcare team.
• Attention to detail and strong analytical skills.
• Familiarity with hospital information systems (HIS) is an advantage.

Pay: Up to ₹25,000.00 per month
