Job Description

Role & responsibilities

Daily audit of prescription and recommendation.
To check error-prone abbreviations in the prescription.
Adverse drug reaction/ Medication Error audit.
Reporting to PVPI.
Blood Transfusion Reaction/ Metal Implant Reaction audit and report to be sent to the Quality department.
Promoting safer and more effective use of drugs.
Rational introduction and use of new and expensive medications.
Training of healthcare staff in promoting the rational use of medicine.
Data Validation of Pharmacy Indicators
Medicine and Consumable audit
Narcotic Medicines Storage, usage and return of narcotics.
Help in Pharmacy Manual and formulary documentation.
To check medicine reconciliation
Active participation in Pharmacotherapeutic and HIC committee.
Organizing Pharmacotherapeutic committee meeting.
Coordination in antibiotic stewardship programme.
Antibiotic utilization audit
Outcome of antibiotic reporting
