Clinical Documentation Auditor

As a Clinical Documentation Auditor at our growing team in Kochi, you will be responsible for ensuring the accuracy, clarity, and compliance of clinical documentation across medical records. Your role will involve the following key responsibilities: - Review clinical documentation in medical records for accuracy, completeness, and clarity. - Audit charts to ensure compliance with industry standards, regulatory requirements, and organizational policies. - Identify documentation gaps and provide feedback to healthcare providers and coding staff. - Ensure all diagnoses and procedures are clearly and accurately documented to support correct coding. - Prepare audit reports with findings, recommendations, and corrective actions. - Conduct regular internal audits to assess documentation practices and maintain quality standards. - Collaborate with clinical teams to improve documentation processes and support quality improvement initiatives. To qualify for this role, you must meet the following qualifications: - Completion of a Nursing degree (B.Sc Nursing, GNM, or equivalent) is mandatory. - Must be a Registered Nurse (RN) with valid credentials. - Prior experience in medical transcription or clinical documentation review is required. - A minimum of 1-3 years of experience in clinical documentation, chart review, medical scribing, auditing, or a related role within a clinical or BPO environment.
