Job Summary:
The Clinical Assistant (MRD) is responsible for the management, organization, and processing of patient medical records in a healthcare facility. The role involves maintaining confidentiality, ensuring records are up-to-date, accurate, and securely stored, and making them readily accessible to authorized personnel when needed.
Key Responsibilities:
v Collect, organize, and maintain patient medical records both in physical and electronic formats.
v Ensure the accuracy and completeness of patient information, including diagnostic reports, treatment history, and other relevant documentation.
v Update and file patient records promptly after consultations, treatments, or diagnostic tests.
v Handle sensitive patient information with strict confidentiality and in compliance with hospital policies and healthcare regulations (e.g., HIPAA, GDPR).
v Safeguard medical records from unauthorized access or damage.
v Input data into the electronic medical records (EMR) system.
v Ensure data integrity and follow up with healthcare professionals to correct any inconsistencies or incomplete entries.
v Respond to requests for patient medical records from authorized personnel such as doctors, nurses, or administrative staff.
v Process external requests for records from insurance companies, legal firms, or other healthcare institutions, ensuring appropriate permissions are obtained.
v Retrieve and deliver patient records as requested for medical reviews or patient care.
v Assist in the transfer of medical records between departments or healthcare providers when necessary.
v Ensure that records are properly stored and archived to meet organizational and regulatory standards.
v Conduct periodic audits to ensure that medical records are up-to-date and complete.
v Ensure compliance with healthcare regulations and organizational policies related to medical record management.
v Coordinate with clinical and administrative teams to ensure smooth access to patient records.
v Serve as a liaison between the medical records department and other departments in the hospital.
v Occasionally interact with patients regarding their medical records, including responding to inquiries about record availability or providing copies of records as needed.
v Good verbal and written communication skills for interaction with staff, patients, and external organizations.
v Uphold the highest standards of confidentiality and ethical handling of sensitive patient information.
