Management of Medication Usage

a) To overview proper indenting of medication

b) To overview proper inventory & stock management

c) To ensure safe medication dispensing process

d) To advice on proper storage of medicines & other pharmaceutical /surgical products

e) To closely work with all the stakeholders, i.e., nursing team, clinicians, pharmacy team to ensure a safe environment for proper utilization of medication.

f) To perform weekly narcotic Audits.

g) To coordinate the PTC as a convener/member secretary.

h) To assess & approve the non-formulary drugs request in the hospital.

2. Medication Safety

a) To analyze the causes of medication errors and advise on mitigation.

b) To monitor and advise on the management of ADRs.

c) To conduct Pharmacovigilance, Materiovigilance, program in the hospital.

d) To overview the process of drug recall during any incompatibility seen in the drug.

3. Antimicrobial Stewardship

a) To involve in the hospital antimicrobial stewardship program as an important KOL

b) To work collaboratively with microbiologist(s) and the laboratory team in developing antimicrobial toolkits and safe prescription and administration of antimicrobials

c) To conduct regular antibiotic audit.

d) To ensure the proper use of restricted antibiotics.

4. Policy making on the medication management in the hospital.

a) Collaborate with all the stakeholders of the hospital in formulating and implementing the medication usage and management policies.

b) To formulate/edit/implement the hospital medication policies as per NABH (MOM) Chapter

5. Hospital Accreditation & Quality:

a) To collaborate with the quality department in the accreditation process

b) To share the safety data and monthly KPI's to the quality department

c) To perform clinical audit on a regular interval

d) To manage the MOM chapters of NABH & MMU chapters of JCI:

• Pharmacy services and usage of medication is done safely
• The Organization develops, updates and implements a hospital formulary.
• Medications are stored appropriately and are available where required.
• Medications are prescribed safely and rationally.
• Medication orders are written in a uniform manner.
• Medications are dispensed in a safe manner.
• Medications are administered safely.
• Patients are monitored after medication administration.
• Narcotic drugs and psychotropic substances, chemotherapeutic agents and radioactive agents are used safely.
• Implantable prosthesis and medical devices are used in accordance with laid down criteria.
• Medical supplies and consumables are stored appropriately and are available where required.

6. Administrative and miscellaneous roles:

a) To lead the team of Clinical Pharmacist and supporting staff

b) To ensure the proper functioning of the department

c) To attain various meeting wherever invited in/out of the hospital

d) To provide drug/poison information services to other healthcare workers

e) To guide/mentor the interns/trainees

f) To conduct/educate other healthcare workers on safe & effective uses of medication

g) To perform other roles and responsibilities to be assigned by the hospital administration from time to time.
