Immediate need for a talented Senior LTSS Service Care Manager RN for a 06+months contract with possible 2 Hire opportunity and long-term potential in San Antonio TX (Onsite). Your responsibilities will include completing SAI assessments at member homes, telephonic outreaches, care coordination, following up on requests for DME, Therapies, provider services, 28-day follow-ups, documentation, and forms. It is crucial to follow work processes and complete assessments, outreaches, and follow-ups within specified turnaround times. On your first day, you will set up equipment and undergo training. Key Requirements and Technology Experience: - Key Skills in Pediatrics and Home Health - RN with pediatrics experience - Proficiency in Word, Excel, Outlook, and Teams - Strong communication skills - Education/Certification: BSN or ADN - Graduate from an Accredited School of Nursing or Bachelors degree in Nursing with 2-4 years of related experience Our client in the Healthcare Industry is actively interviewing for this position.
