Job Description

Role & responsibilities

Review and analyze medical records, physician documentation, and diagnostic reports for E/M and Radiology services.
Assign accurate CPT, ICD-10-CM, and HCPCS codes based on documentation and coding guidelines.
Validate medical necessity and ensure compliance with payer-specific rules.
Work closely with physicians and clinical staff to clarify documentation and resolve coding discrepancies.
Maintain up-to-date knowledge of coding regulations, CMS guidelines, and industry changes.
Assist in audits and quality assurance to ensure coding accuracy and reduce denials.
Meet productivity and accuracy standards set by the organization.

Required Skills & Qualifications:

Certification:
CPC, COC, or CCS-P (AAPC or AHIMA).
Strong knowledge of
E/M coding guidelines
,
Radiology CPT codes
, and ICD-10-CM.
Familiarity with
Medicare and commercial payer rules
.
Proficiency in medical terminology, anatomy, and physiology.
Excellent attention to detail and analytical skills.
Ability to work independently and meet deadlines.
