1. Core Responsibilities

A. Review Clinical Documentation

• Read

  anesthesia records

  , operative notes, pre-anesthesia evaluations, PACU notes, and surgeon documentation.
• Verify

  ASA physical status

  , type of anesthesia, procedures performed, and time logs.
• Ensure documentation supports medical necessity and coding choice.
  

B. Assign Appropriate Codes

CPT / ASA Code Assignment

• Select correct

  CPT anesthesia codes

  linked to the surgical procedure.
• Apply

  ASA Physical Status Modifiers

  (P1P6).
• Ensure proper use of

  additional anesthesia modifiers

  :

• QS

  – Monitored anesthesia care (MAC)

• QK/QY/QX/QZ

  – Medical direction or supervision

• P1–P6

  – ASA status

• AA/AD

  – Anesthesiologist vs. CRNA services

ICD-10-CM Diagnosis Coding

• Identify principal and secondary diagnoses supporting anesthesia need (e.g., comorbidities like hypertension, diabetes, COPD).
  

C. Calculate & Verify Anesthesia Time

• Review anesthesia start/stop times.
  
• Convert to billable time units based on payer rules.
  
• Identify discrepancies or missing time entries and query providers.
  

D. Apply Payer-Specific Rules

• Follow Medicare, Medicaid, commercial payer rules for:
  
• Base units
  
• Time units
  
• Modifiers
  
• Supervision vs. medical direction
  
• Bundling/unbundling requirements
  
• Check NCCI edits and LCD/NCD guidance when applicable.
  

E. Compliance & Audit Responsibilities
