Patient Coordinator

As a Patient Care Coordinator, your primary responsibility will involve coordinating patient care schedules with doctors and other healthcare professionals. You will be tasked with communicating effectively with patients regarding their appointments, care plans, and any changes in treatment. It will also be essential for you to document patient health information, including health histories and insurance details, accurately. In this role, you will play a crucial part in assisting patients in understanding their health conditions and treatments. Ensuring that all necessary patient information is collected and maintained in patient files will be a key aspect of your duties. You will work closely with healthcare providers to develop individualized patient care plans and assist patients with the coordination of referrals and insurance pre-certifications.
