Looking for HCC Coding(Team Lead & Manager)
*Looking for detail-oriented coder specializing in Hierarchical Condition Category (HCC) / risk-adjustment coding and support revenue integrity across our healthcare operations.
*Review patient medical records/chart documentation to identify diagnoses and/or conditions that map to HCC risk adjustment models.
*Assign correct ICD-10-CM codes (and CPT/HCPCS where applicable) based on documentation. *Maintain knowledge of official coding guidelines, payer specific rules, regulatory requirements, and risk-adjustment methodologies. *Collaborate with clinical documentation improvement (CDI) teams or providers to clarify documentation if required.
