Job Description

Responsibility

Patient Monitoring:
Conduct regular home visits to assess vital signs, monitor for changes in health, and document patient progress.
Medication Management:
Administer prescribed medications and insulin, and manage complex treatments such as intravenous (IV) therapy.
Wound Care:
Inspect and dress wounds, and monitor the healing process to prevent infection.
Patient Education:
Instruct patients and their families on necessary procedures, nutrition, pain management, and how to use medical equipment.
Care Plan Development:
Collaborate with physicians and other healthcare providers to develop and implement patient-specific care plans.
Documentation:
Maintain accurate and detailed medical records of patient history and care activities.
Emotional Support:
Provide compassionate support and reassurance to patients and their families, addressing their concerns and questions.
Assistance with Daily Living:
Help with personal care tasks such as bathing, grooming, and hygiene, and support with the use of assistive devices.
