Healthcare Support Worker - Hospital at Home

As a Health Care Support Worker (HCSW) in the Hospital at Home service, you will be an essential part of the team providing clinical care to patients in their own home or care setting. Your role is crucial in ensuring safe, effective, and high-quality care delivery in the patient's home environment. Key Responsibilities: - Liaise with various professionals and agencies on a daily basis, including GP practices, local authority teams, and the voluntary sector. - Work as part of a multidisciplinary team to carry out personal care duties for patients under the supervision of a registered nurse. - Look after a group of frail, elderly patients and deliver care in a home environment. Qualifications Required: - Previous experience in a healthcare setting is preferred. - Good communication skills and the ability to work effectively in a team. - Willingness to work flexible hours, including weekends. Please note that the Hospital at Home service operates 7 days a week from 0800-1800. As an HCSW, your working hours will be Monday to Friday with weekend shifts included.
