HCC Coders

We are seeking an experienced Medical Coder specializing in HCC (Hierarchical Condition Category) Risk Adjustment Coding to join our team. The ideal candidate will have a strong background in ICD-9-CM coding, medical terminology, and thorough knowledge of HCC coding guidelines.

Key Responsibilities:

Review and analyze medical records to accurately abstract HCC codes in compliance with official coding guidelines and client-specific requirements.

Perform detailed chart reviews, including

provider documentation, inpatient, and outpatient records to identify valid HCC diagnoses (Part C & Part D).

Ensure accurate and complete capture of diagnosis codes from approved medical record documentation sources.

Adhere to all CMS, AAPC, and AHIMA coding standards and updates.

Consistently meet or exceed established productivity and accuracy benchmarks.

Communicate effectively with team members and reporting managers to resolve coding-related queries in a timely manner.

Complete administrative and documentation tasks as required by management.

Maintain strict adherence to HIPAA and data privacy standards.

Qualifications:

AAPC or AHIMA Certified Coder (e.g., CPC, CRC, CCS-P, etc.)

Proficiency in ICD-9-CM coding (knowledge of ICD-10-CM is a plus)

Strong understanding of medical terminology, anatomy, physiology, and disease processes

Familiarity with official HCC coding and chart review guidelines

Excellent attention to detail and analytical skills

Flexible and adaptable to meet client goals and evolving requirements
