Emergency Patient Care Coordinator

We are seeking an Emergency Patient Care Coordinator to serve as the central point of contact for patients in the emergency department. You will coordinate care plans, facilitate communication between patients, families, and healthcare teams, and ensure patients receive efficient, compassionate care.

Key Responsibilities:

• Serve as a liaison between patients, families, and medical staff.
• Coordinate and monitor individualized care plans, appointments, and referrals.
• Educate patients and families on conditions, treatments, and medications.
• Advocate for patients’ needs and rights.
• Collaborate with interdisciplinary teams to ensure cohesive care.
• Manage administrative tasks including scheduling, records, and insurance support.
• Support patient flow and ensure adherence to hospital standards.

Job Type: Full-time

Pay: ₹20,000.00 - ₹25,000.00 per month
