Job Description: As a Personal Care Manager at Citraverse, you will play a pivotal role in providing personalized care and companionship( 24/7 )to elderly individuals in the comfort of their homes. Responsibilities: · Provide personal care assistance to elderly clients, including bathing, grooming, dressing, Diaper change and toilet needs. · Assist with mobility, transfers, and ambulation as needed. · Administer medication according to prescribed schedules and instructions. · Assist with feeding if required. · Engage clients in meaningful activities and companionship to promote social interaction and mental stimulation. (Gardening, Reading, Music, Stimulation games etc) · Maintain a safe, clean and hygienic environment for clients, ensuring their comfort and well-being at all times. · Monitor and report any changes in clients' health or behavior to the designated care coordinator. · Accompanying clients on outings and medical appointments. · Engaging clients in conversing and walks. · Massage therapy and assist clients in physical therapy exercises. · Collaborate effectively with healthcare professionals, family members, and other caregivers to ensure coordinated care delivery. · Update daily care plan in the platform. · Identify red flags and inform the healthcare provider/family.
