Care Navigator

Role Overview: As a Care Navigator at Rethink Mental Illness, you will be an integral part of the Prisons and Criminal Justice Directorate Team, working to provide support to individuals severely impacted by mental illness. Your role will involve ensuring continuity of care and successful reintegration into the community for individuals with health vulnerabilities upon their release from prison. By reconnecting individuals with health services and managing their wellbeing through a coaching framework, you will contribute to reducing health inequalities and reoffending rates. Key Responsibilities: - Provide a seamless package of support to individuals with health vulnerabilities upon their release from prison - Reconnect individuals with health services to reduce health inequalities and reoffending - Work within integrated mental health teams and community settings to provide 1:1 and group assessment and support - Use a coaching framework to manage wellbeing in the pre and post-release period - Collaborate with a range of professionals to ensure the highest quality of care for individuals with mental health needs within the prison setting - Engage in travel as required for service delivery, with expenses covered Qualifications Required: - Experience in providing support to individuals with mental health needs, preferably in a criminal justice or similar setting - Strong communication and interpersonal skills - Ability to work autonomously and collaboratively within a team - Understanding of health inequalities and reoffending issues - Commitment to diversity, equity, and inclusion principles Company Details: Rethink Mental Illness is committed to creating a diverse, inclusive, and anti-racist workplace.
