Transplant Coordinator -Nurse

• Transplant nurse coordinator works closely with each patient from the time of evaluation before transplantation through approval for transplant waiting list.
• Transplant coordinator Nurse work with living donor and recipient to ensure each patient has thorough medical, physical and mental health work up and is adequately prepared to pursue living donation.
• Nursing transplant coordinator plays a major role in the provision and advancement of the care of organ recipients and donors as well as their families.
• Assess the factors influence the provision of transplant nursing care that requires specialized care based upon expert knowledge and skills from experienced transplant nurses.
• Providing collaborative care as members of an transplant team of specialists
• To work with the hospital information system and other information technology resources and software as may be assigned on a periodic basis from time to time as per attached appendix
• Create donor awareness.
• Education of the patient, their family and support system, and the community along with social worker
• Provides education to healthcare providers in hospital and community regarding transplant issues
• Participates in evidence based nursing to improve transplant nursing practice
• Teaching team members, community partners, and/or deceased donor families and others about the donation process
• Coordinating the process of organ retrieval procedure.
• Facilitating allocation to the recipientas per ZTCC waiting list with coordination with social worker.(Transplant coordinator)

Pre-transplant care of recipient

• Focus on managing issues related to organ failure to ensure the patient remains in optimal health, enabling the patient to proceed with transplantation when a suitable organ is available.
• Managing the potential recipient’s evaluation to determine candidate status
• Assessing and monitoring the candidate’s health throughout the process and while on the waiting list so that the candidate is in optimal physiological, psychological, and psychosocial health at the time of transplant.
• Supporting the transplant candidate during the potentially long wait for transplant by developing and enhancing their coping skills.
• Providing anticipatory teaching about post-transplant issues including: medications, signs and symptoms of infection and rejection, chronic care needs, post-transplant follow-up routines, and potential psychosocial issues.

For Cadaver donors:

• Check the consent form of cadaver donor such as Form no:8 (declaration and consent for organ donation) Form no: 10 i.e Brain stem death certification.
• Inform to Transplant surgeons about first apnea test and the approximate time of second apnea test and results.
• Inform to OT after first apnea test if positive, to prepare OT for retrieval and transplant procedure. Inform to supportive dept. such as CSSD. BLOOD BANK.SODEXO.(food & beverages) about emergency transplant.
• Check availability of HTK and ice cold NS 1.lit. in OT.
• Check the consents of recipients for surgical procedure, and high risk
• Assist to Surgeons or Nephrologists for organ perfusion procedure.

For live donors:

• Ensure consent of surgical procedure
• Approval from local authority committee/ DHS
• Ensure that the Donors file of investigation reports is arranged appropriately.

Post Transplant Care

• Help to achieve the highest level of wellness possible
• Provide Neutropenic care
• Immunosuppressant adherence
• Tacrolimus levels monitoring
• Providing emotional support
• Patient education including medications & adherence to treatment, signs & symptoms of rejection, chronic care needs, potential post transplant complications, follow up & healthy living issues post transplant
