Assures continuation of therapeutic plan following discharge by designing and instructing client families and caregivers in home exercise programs, recommending or providing assistive equipment. Documents client care services by charting in client and department records such as CRM, client record files, etc. To educate a client family about how to accommodate and care for the client by giving advice and arranging support for family members, caretakers and clients To undertake general administrative tasks for example writing reports, maintaining records and case notes. Involve the multi-disciplinary case discussion on a regular basis and documentation of all cases. Licensed or certified as a Mental Health Counsellor, Counselling Psychologist, or equivalent credential preferred. Proficiency in evidence-based therapeutic approaches such as CBT, DBT, Mindfulness-Based Therapies, and Trauma-Informed Care. Experience in at least two types of therapy (individual, family, child, or couple) preferred. Excellent listening, empathy, and rapport-building skills with a client-first approach. Ability to work independently and collaboratively within a multidisciplinary team environment. Strong attention to detail, time management, and documentation practices.
