Staff Nurse - Homecare

Role Overview: You will be responsible for delivering high-quality nursing care to patients in their homes. This includes conducting comprehensive nursing assessments, developing personalized care plans, administering medications, monitoring patients" conditions, providing various nursing procedures, educating patients and caregivers, maintaining clinical records, ensuring infection control and safety standards, collaborating with healthcare professionals, and offering emotional support to patients and families. Key Responsibilities: - Conduct comprehensive nursing assessments of patients in their home environment. - Develop and implement personalized care plans in collaboration with the healthcare team and family members. - Administer medications and treatments as prescribed, ensuring safety and proper documentation. - Monitor patients" conditions and promptly report changes to physicians or case managers. - Provide wound care, catheter care, IV therapy, and other nursing procedures as needed. - Educate patients and caregivers on disease processes, medications, and self-care techniques. - Maintain accurate and up-to-date clinical records and reports. - Ensure infection control and safety standards are maintained at all times. - Collaborate with other health professionals (physicians, therapists, social workers) to optimize patient care. - Support patients and families emotionally, offering guidance and assistance with accessing community resources when needed. Qualification Required: - ANM / DGNM/ BSc (N) Additional Details: You may be required to conduct home visits in a variety of residential settings with flexible work hours, including potential evenings, weekends, or holidays based on patient needs.
