Job Description

The Field Outreach Worker isresponsible for engaging with vulnerable populations such as the homeless andelderly through proactive field visits, building trust to ensure access to essentialhealthcare, social services, and rehabilitation programs.
This role involves conductingassessments, maintaining accurate case records, developing tailored interventionplans in collaboration with medical and social work teams, and facilitatingconnections to necessary services including mental health support, socialsecurity benefits, and emergency care.
Additionally, the position includes supportingreintegration efforts, family tracing, and providing crisis interventionthrough psychological first aid, while also promoting community awareness andadvocacy by collaborating with local stakeholders.

Outreach & Engagement: Identify and connectwith homeless individuals, elderly persons, and vulnerable groups through fieldvisits. Build trust and encourage participation in services. Coordinate OP, IP,rescue and mobilization.

Assessment & Case Management: Conduct needs assessments,maintain case records, and develop intervention plans with the medical andsocial work teams.

Linkage to Services: Facilitate access tohealthcare, rehabilitation, social security benefits, and other essentialservices.

Reintegration & Family Tracing: Supportfamily tracing, safe resettlement, and follow-up for stability.

Community Awareness & Advocacy: Conduct awarenesssessions and collaborate with stakeholders to strengthen support networks.

Crisis Intervention: Provide emergency support, psychologicalfirst aid, and referrals for urgent medical or psychiatric care.

Networking: Build rapport with differentorganizations and government agencies like the police. Build networks witheducational institutions and service providers.
