We are seeking a compassionate, detail-oriented Patient Coordinator to serve as a key liaison between patients and the Hospital. The Patient Coordinator will be responsible for ensuring an excellent patient experience from pre-admission to post-discharge, addressing patient concerns, coordinating services, and driving overall satisfaction across touch points. Key Responsibilities: Serve as the primary point of contact for patients and their families throughout their care journey. Monitor and proactively improve patient satisfaction by addressing issues, collecting feedback, and ensuring timely resolutions. Coordinate patient appointments, admissions, discharges, and follow-ups in collaboration with clinical teams. Provide patients with clear, accurate information regarding procedures, hospital policies, billing, and care timelines. Track and analyse patient feedback, complaints, and satisfaction scores; escalate unresolved issues to relevant departments. Educate patients on services available and advocate for their needs within the hospital. Ensure that all interactions reflect a high standard of empathy, professionalism, and responsiveness. Collaborate with nursing, medical, and administrative staff to streamline patient processes and eliminate service gaps. Support the implementation of patient satisfaction surveys and assist in quality improvement initiatives.
Plays a vital role in maintaining accurate and up-to-date patient records, administrative tasks, and overall office efficiency. Key Responsibilities: Data Input and Management: Accurately inputting patient information, medical history, treatment details, and other relevant data into digital databases. Record Maintenance: Maintaining and updating patient records, ensuring data accuracy and consistency. Clerical Support: Assisting with general office tasks such as filing, scanning, and printing documents. Communication: Communicating with other staff members to clarify data inconsistencies or obtain necessary information. Report Generation: Generating reports and summaries from the database as needed.
Communicate with patients to get detailed case histories; Examine patients to detect visual disorders. Check symptoms of general health conditions (e.g. diabetes) Sending patients to ophthalmologist with prescriptions for spectacles or contact lenses. Check prescribed specs/lenses in order to correct visual disorders. Offering help and advice for patients choosing frames and lenses; Writing referral communications to ophthalmologists. Provide vision therapy and low vision rehabilitation. Perform routine refraction, IOP, Perimetry, Keratometry, Biometry, Orthopic Evaluation, Orthoptic Exercise, Squint Evaluation and Diplopia charting. Conduct eye exams using optometric equipment and general diagnostic interpretation. Diagnose and treat ocular disease. Prescribe, fit, and adjust eyeglasses, contact lenses, and other vision aids. Deliver outstanding customer service through all patient interaction
