As a Registered Nurse in our Home Health team, you will provide compassionate and high-quality care to patients in their homes, ensuring their well-being and comfort. You will play a crucial role in assessing patient conditions, implementing care plans, and delivering patient care within our organizational philosophy and policy. Your responsibilities will also include providing ongoing education and support to patients and their families, performing admission processes, and managing cases effectively. Utilizing thorough and timely electronic documentation and interdisciplinary communications will be essential in your daily tasks. Key Responsibilities: - Assess patient conditions and implement care plans - Provide ongoing education and support to patients and their families - Deliver patient care within organizational philosophy and policy - Perform admission processes and case management responsibilities - Utilize thorough and timely electronic documentation and interdisciplinary communications Qualifications Required: - Certified Home Care experience: 1 year - Case Management experience: 1 year - Experience in implementing care plans and Patient Care: 2 years - RN License - BLS Certification Please note that you will have the opportunity to work full-time, travel nursing, or on a contract basis.
