RN- Case Manager

Job Description: As an RN-Case Manager in this contract position, you will be expected to start immediately and float to the Laguna Facility. Your primary responsibility will be to manage a caseload with a ratio of 1:25 patients, drawing on your 2 years of acute medical Care Manager experience in performing Care Coordination. Please note that first-timers are not accepted for this role, as it requires travel experience. You will also be part of the weekend rotation schedule. A California state license is required for this position, and pending licenses are accepted as long as you are licensed upon the start date. There are no Return to Work (RTW) restrictions beyond 7 days. Key Responsibilities: - Manage a caseload with a patient ratio of 1:25 - Perform Care Coordination utilizing your 2 years of acute medical Care Manager experience - Float to the Laguna Facility - Participate in weekend rotation schedule - Obtain and maintain a California state license - Ensure compliance with travel experience requirement Qualifications Required: - 2 years of acute hospital case management experience - BLS Certification - Ability to commute to Mission Viejo, CA 92691 - Willingness to relocate to Mission Viejo, CA 92691 before starting work In addition, this position is based in an Acute Care Facility where the scrub color is Turquoise or Business Casual.
