As a Nursing Coordinator, you will be responsible for administering skilled nursing care to patients in their place of residence based on physician orders and care plans. Working under the direction and supervision of the Director of Clinical Services/Nursing Supervisor, you will play a crucial role in evaluating and meeting the nursing needs of patients. You should hold a BSC (Nursing) or GNM from a state-approved school of practical nursing and possess a current state license. A minimum of three years of nursing experience is preferred, along with the ability to exercise initiative and independent judgment. Your responsibilities will include conducting initial evaluation visits, developing and revising care plans, providing services as per the care plan, recruiting and training Caregivers, documenting patient visits, coordinating services, counseling patients and families, participating in in-service programs, processing orders, supervising nursing personnel, and conducting patient care conferences. Additionally, you will be required to be available for on-call duty during nights, weekends, and holidays, as assigned. As a Nursing Coordinator, you will report to the Operational Manager/Nursing Supervisor and will supervise Licensed Practical Nurses and Trained Nursing Attendants. This position offers full-time, permanent employment opportunities with benefits such as cell phone reimbursement and work-from-home options.
