NMP+ under EpiC India grant is implementing life-saving activities for HIV care and treatment under GHSD guidance supported by United States Department of State (DoS). The treatment navigators- NCD located in the Pune,Thane and Alibag will be placed in selected ART centers managed by Maharashtra State AIDS Control Society and respective District AIDS Prevention and Control Units (DAPCUs). They will be responsible for below-mentioned activities in coordination with ARTC staff. They will be directly reporting to Program Manager NCD in NMP+, based in Pune. Job Responsibilities Ensure deliverables of the approved site-wise activities and targets screening of NCD are achieved with support of facility staff and technical input from Technical Advisor-HIV in Maharashtra. Coordinate with ART Centers (ARTCs) for implementing the service delivery improvement activities related to screening for NCD among eligible PLHIV clients. Support ART center staff in screening PLHIV who are more than 40 years for NCDs (diabetes, hypertension, cardiovascular health, mental health and cancer) and link those who need further services with respective departments in the same facility or linking them to the facilities where they can receive required services. Support the roll-out of the NCD self-care literacy materials in the ART centers through patient literacy sessions by delivering self-care messages tailored to the needs of PLHIV, promoting positive health-seeking behaviors, treatment adherence, and mental well-being Facilitate the ART Center staff in demand generation activities Support the ART center staff in data recording, tracking clients for follow-up and sharing the analysis monthly with the center staff. Ensure all compliances related to data both from MSACS/NACO and DoS. Ensure that the data confidentiality and data sharing norms are practiced. Online reporting mechanism is used to maintain the required information for NCD related activities. Education Qualification/Required Skills & Experience Preference will be given to the candidates from Social work background. Minimum of 3 years of experience in working with PLHIV clients is mandatory. Knowledge about challenges faced by people living with HIV is preferred. Knowledge about the functioning of ARTCs, ICTCs and CSC partners is required. Working knowledge of online data entry, SMS and WhatsApp based communication is preferred. People living with HIV and women candidates fulfilling above criteria will be preferred. Preferably excellent in oral and written communication skills in English, Marathi and Hindi. Self-motivated and committed individuals to support PLHIV and their partners, who can influence PLHIV individuals to take care of their health and lead a quality life. Having two wheeler and own laptop is highly preferable.
