Requirements: - Title : Medical coder - Certification: CPC/COC/CCS/CIC (Any one) - Location: Noida 126/Hyderabad (5 days onsite) - Shift: 12PM-9PM IST (Both Side Cab) Job Description: We are looking for enthusiastic and detail-oriented freshers to join our Medical Coding team, primarily supporting Inpatient and Outpatient Coding projects. You will be responsible for reviewing clinical documentation and assigning accurate codes as per ICD-10-CM, CPT, and HCPCS guidelines. In addition to core coding responsibilities, selected candidates may also be involved in EMR Migration projects, assisting in the accurate transfer and validation of patient records from legacy systems to modern EMR platforms. Key Responsibilities: Review, analyze, and interpret the complete electronic medical record for each patient encounter or admission. Assign appropriate ICD-10-CM, CPT, and HCPCS codes, ensuring compliance with official coding guidelines. Determine and assign principal and secondary diagnoses and procedure codes. Collaborate with Clinical Documentation Integrity Specialists (CDIS) and perform physician queries when documentation is incomplete or unclear. Participate in DRG reconciliation and coding audits (internal/external). Ensure documentation supports severity of illness, risk of mortality, and compliance requirements. Adhere strictly to HIPAA regulations and maintain data confidentiality. Assist in the extraction, mapping, and validation of patient data during EMR migration. Work collaboratively with quality, auditing, and project teams for continuous improvement and accuracy. Candidate Requirements: Strong foundational knowledge of anatomy, physiology, pharmacology, and pathophysiology. Certification in CPC, COC, CIC, or CCS (mandatory for production work). Basic familiarity with EMR/EHR systems and healthcare workflows (training will be provided). Excellent attention to detail and accuracy. Proficiency in MS Excel (sorting, filtering, formulas).
