Key Responsibilities: Conduct comprehensive nursing assessments of patients in their home environment. Administer medications and treatments as prescribed, ensuring safety and proper documentation. Monitor patients’ conditions and promptly report changes to physicians or case managers. Provide wound care, catheter care, IV therapy, and other nursing procedures as needed. Educate patients and caregivers on disease processes, medications, and self-care techniques. Maintain accurate and up-to-date clinical records and reports. Ensure infection control and safety standards are maintained at all times. Collaborate with other health professionals (physicians, therapists, social workers) to optimize patient care. Support patients and families emotionally, offering guidance and assistance with accessing community resources when needed. Working Conditions: Home visits in a variety of residential settings.
